Lew "Chub" Ray Forman Sr. 83, of Cherryvale, died Saturday, August 22, at home surrounded by family.

Lew was born October 28,1942, in Independence, Kansas, son of the late William and Bernice (Keath) Forman.

Lew attended Zion Lutheran School, I.H.S. and I.C.C. He received his GED in 1981. He attended SEK VO-TECH and graduated in 1986.

September 1960, he enlisted in the United States Navy. His service took him around the world, as he deployed on the USS Nimitz. He was stationed in Vietnam; Naples, Italy; San Diego, CA; Fort Lee, VA; and Corpus Christi, TX. On May 30,1980, Lew retired as chief petty officer after 20 years of service. He was proud of his Navy life.

On March 4,1967, he was united in marriage to Barbara Jean (Hamilton), who survives. Added to this marriage were 7 children. Lew was a member of the Cherryvale Bible Holiness Church, where he attended since 1980 and served as Sunday School teacher and board member for several years.

Lew loved working with wood and figuring out how to make things himself. He enjoyed tinkering with small engines, gardening, and watching the cows out the window. He looked forward to surprise visits from the grandchildren and lately, the greats!! He enjoyed the phone calls asking him to come to the restaurant for breakfast.

Lew is survived by his wife Barbara, of 59 years. Children, Shirley Sexton (Cherryvale), Cora & Mike Haines (Wichita), William Forman (Cherryvale), Lew Jr. & Jasmin Forman (Cherryvale), Donald & Teresa (VA), grandchildren, Xavier & Lauren Haines (TX), Gavin & Hannah Haines (Wichita), Landon Haines (Wichita), Haiden Haines (TX), Natalee Davis & Hayden Palmer (Coffeyville), Camdyn & Houston Greenfeather (Cherryvale), Blaze Forman & Maggie Catlin (Cherryvale), Kadynce Forman (Cherryvale), great grandchildren, Costyn, Crosslee, Bodhi, 3 foster children, and 1 expected in February; 1 brother Wayne Forman, plus many bonus family and friends.

Lew is proceeded in death by his parents, William & Bernice Forman, son, Jeffery Robert, brother Harry, 1 pre-born child, many nieces, nephews, aunts, uncles, and cousins.

The family wishes to thank the Gentiva Hospice nurses and aides who gave such WONDERFUL care to him!! A big thanks to Rebecca Miller "Cook" as Lew called her), with Cornerstone Caregiving, for all the "at command breakfasts" and care given.

A visitation for Lew will be held on Friday, August 28, 2026, from 6-8 p.m. at the Bible Holiness Church, 606 Park Street, Cherryvale, Kansas 67335.

A Celebration of Life Funeral Service will be held Saturday, August 29, 2026, at 10:30 a.m., also at the church.

Interment will take place immediately following the service at Fairview Cemetery in Cherryvale, Kansas with full military honors under the Independence Honor Guard.

In lieu of flowers, memorial contributions can be given to the Cherryvale Bible Holiness Church, and they may be left or mailed to Wickham Family Funeral Home, 216 East 4th, Cherryvale, Kansas 67335.
